Physical counseling records formerly in the keeping of Southwest Counseling Associates are now stored with Morgan Records Management, a firm specializing in HIPAA-compliant protection of private information.
Clients or authorized representatives of clients may request copies of counseling records by going to the Morgan website using the following link:
Clients are encouraged to contact the therapist who created the records before requesting copies from Morgan. Please contact the therapist if you have any questions or difficulties.
